Fusion Adaptive Fitness provides adaptive fitness services for children, teens and adults in Port Jefferson Station, NY.

What Is Fusion Adaptive Fitness?

Fusion Adaptive Fitness is a fitness program based in Port Jefferson Station, NY.

We provide individualized and small-group exercise options for children, teens and adults who may benefit from a more flexible coaching approach.

Training may be adapted around:

  • pacing
  • exercise experience

Fusion focuses on fitness, coaching, exercise, strength, conditioning and movement.

Adaptive Personal Training

One-on-one fitness coaching built around the individual athlete. Training may include strength, conditioning, balance, coordination, movement skills and exercise confidence.

Who We Work With

Who May Use Fusion Adaptive Fitness?

Fusion works with children, teens and adults who may benefit from individualized fitness coaching or exercise modifications.

This may include some athletes with:

  • autism
  • intellectual disabilities
  • developmental disabilities
  • mobility differences
  • coordination challenges
  • other support needs

We do not assume every person with a particular diagnosis is automatically an appropriate fit for every Fusion program.

Each athlete should be considered individually.

Considered Individually

No diagnosis automatically qualifies or disqualifies an athlete. We evaluate fit based on the person, the program and our current facility.

Vendor Documentation

What Information Can Fusion Provide?

If a participant's Self-Direction team is evaluating Fusion, we can provide reasonable documentation needed to understand the service.

If your Fiscal Intermediary uses a specific vendor form or onboarding process, send it to us and we can review what is required.

For Support Brokers

Support Brokers help participants with Budget Authority develop and manage Self-Direction budgets.

If a participant is interested in Fusion Adaptive Fitness, we can help by providing clear service information so the broker can evaluate how the expense may relate to the participant's goals and approved budget.

Information we can provide may include:

  • exact program type
  • service description
  • pricing structure
  • schedule
  • whether the service is one-on-one or group-based
  • whether a class is available to the general public
  • business documentation
  • invoices

Fusion does not determine which Self-Direction funding category should be used. That decision remains with the participant and their Self-Direction team.

We describe the service. The broker decides the category.

Our role is to make the fitness program easy to evaluate against the participant's approved plan.

For Fiscal Intermediaries

For Fiscal Intermediaries

Fiscal Intermediaries may have different procedures for:

  • vendor onboarding
  • direct vendor payment
  • participant reimbursement
  • invoices
  • attendance records
  • membership expenses
  • class expenses
  • documentation

Fusion can work with the participant and FI to provide reasonable business and service information.

If your organization has a standard vendor packet or required forms, please send them to us.

We want the service and payment process to be clear before training begins.

Potential Fitness-Related Self-Direction Categories

Depending on the participant's approved Self-Direction plan, certain fitness expenses may be considered through categories such as:

Health Club / Membership

A qualifying fitness or health-club membership may be relevant when tied to approved health, fitness or community-integration goals.

Community Classes

Exercise classes may be relevant through the Community Class category when current OPWDD requirements are met, including public availability and an integrated community setting.

Individual Exercise Training

Certain individual exercise or physical-education training may be considered when it is tied to the participant's valued outcomes and meets applicable Self-Direction requirements.

Important: Fusion does not decide which category applies. The participant's Support Broker, FI and Self-Direction team should make that determination.

Adaptive Group Fitness Is Not Automatically an OPWDD Community Class

A Fusion group fitness class and an IDGS Community Class are not automatically the same thing.

For a class to fit the OPWDD Community Class category, current OPWDD guidance generally requires that the class:

  • be available to people with and without developmental disabilities
  • take place in an integrated community setting
  • teach a specific subject related to the participant's goals
  • not duplicate another Medicaid State Plan or Waiver service

If a specific Fusion class is being considered for that funding pathway, we can provide information about:

  • how enrollment works
  • who the class is available to
  • class description
  • schedule
  • pricing

The Self-Direction team can then evaluate the class against current requirements.

Not automatically a Community Class.

We provide the class details. Your team determines whether it meets the IDGS Community Class requirements.

How Does Payment Work?

Payment procedures vary by Fiscal Intermediary.

Depending on the participant's approved budget and FI process, payment may involve:

  • direct payment from the FI to Fusion
  • participant or family reimbursement
  • another approved process established by the FI

Fusion does not require families to assume that reimbursement is the only option.

Before paying out of pocket, participants should ask their FI how it wants the expense handled.

If direct vendor payment is preferred, the FI can tell us what onboarding or invoice information it requires.

Check with the FI before paying out of pocket.

Direct vendor payment may be an option depending on the FI's procedures.

Can Fusion Provide Attendance Documentation?

Where appropriate, Fusion may be able to provide reasonable documentation showing participation or services provided.

The exact documentation depends on:

  • service type
  • membership structure
  • class format
  • FI requirements

If a specific format is required, please let us know before services begin whenever possible.

Can Participants Attend With Support Staff?

Some athletes train independently.

Others may attend with:

  • a family member
  • caregiver
  • Community Habilitation worker
  • direct support professional
  • other support person

Whether support staff can remain involved depends on the athlete, program and gym environment.

Fusion does not provide Community Habilitation. The outside support person remains responsible for the support services they provide.

Outside support is welcome where appropriate.

Fusion provides the fitness service; the support person provides their own support service.
